Change only what you know to be wrong, and tell us where you saw the right answer. A person checks every suggestion against the practice's own published information before anything on this page changes.

The name on the sign or the front of their own website, not a shortened version people use locally.

The number that reaches the practice — from their website, their voicemail greeting or the door.

A published address for the practice. Do not enter a personal address for somebody who works there.

The full address including https://. If they have no website, leave this empty rather than linking a social media page that is not theirs.

Street number and name, as the post would be addressed.

Suite, unit or floor, if the practice has one.

The city on their own listing, which is not always the nearest large town.

Two-letter state code.

Five digits, or ZIP+4.

A plain description of the practice. Not advertising copy — promotional text is removed under the Review Content Guidelines.

Optional, but a suggestion we can check is a suggestion we can act on.
